§ 20-29. Application.

  • Each applicant for a permit under this article shall submit to the chief of police or his designee, at least five (5) business days before the proposed date(s) of solicitation, a sworn statement containing the following information:

    (1)

    The name, address, of the applicant and the name, address, age, and sex of each individual soliciting on behalf of the applicant (solicitors).

    (2)

    If the applicant is an organization, the names and addresses of the officers of its governing board.

    (3)

    The percentage, if any, to be paid to the solicitors.

    (4)

    The name of person(s):

    a.

    For whose benefit the solicitation is to be made or, if an organization, the officials of the organization for whose support the permit is asked;

    b.

    The names of the individual(s) who has authority to execute the sworn official statement of accounting of funds described in section 20-28 of this article; and

    c.

    The names of the individual(s) who has authority to determine how the proceeds of the permitted charitable solicitation may be applied or utilized.
